Press START, choose a new game, and follow the opening story about Strepfinger kidnapping Mayor Spryman. The first side-view stage teaches the basic route through Hector's body: move to the right, clear gaps with jumps and a second mid-air jump, line up the pistol before firing at germs, and reach the boss at the end. Later stages switch to the Cel Camino and other first-person routes through blood vessels and nerves; stay in the open lane, dodge traffic and parasitic mines, and keep moving toward the next boss. Level codes let you resume later from the appropriate stage.

Control Pad: move in side-view stages; steer or shift lanes in driving stages; aim when a first-person shooting section allows it. The action buttons perform jump/double jump, pistol fire and acceleration according to the current stage, with the current action shown on screen. START begins the game and opens the in-game menu.
